Referring professionals may use our secure referral form to submit the referral question, patient or client information, and relevant supporting documents.
Once received, our team will review the referral and coordinate directly with the patient or client regarding scheduling and next steps. With appropriate authorization, we can communicate relevant findings and recommendations back to the referring professional.
